چکیده
Ependymoma accounts for 6-12% of all intracranial tumors, but only 6 cases of melanotic ependymoma have been reported. Sellar ependymomas are also extremely rare, with only 5 cases reported in the pituitary fossa. A 26-year-old man presented with an extremely rare case of primary intrasellar melanotic ependymoma manifesting as consistent frontalgia. Magnetic resonance imaging showed a sellar lesion slightly compressing the optic chiasm upwards with various sizes and shapes of significant hypointense areas, initially suspected to be flow voids, in and on the surface of the lesion. Transsphenoidal surgery was performed, resulting in gross total removal of the tumor. Postoperative histological examination disclosed that the tumor cells had round to semioval nuclei without specific structures. Nucleoli were distinct, and frequent brown pigmentations were seen in the cytoplasm. Immunohistochemical examinations revealed diffuse positive reactions to vimentin, glial fibrillary acidic protein, and CAM5.2, and focal positive reaction to HMB-45. Type IV collagen was negative, and epithelial membrane antigen showed dot-like positive reaction at the perinuclear area. Gamma knife surgery was applied, and the patient has been healthy without tumor recurrence for 24 months. This extremely rare variant of ependymoma should be included in the differential diagnosis if melanin production is suspected.

Multiple primary tumors in a single patient are relatively rare when four or more distinct lesions are considered. Herein, we report a case of woman with four different primary tumors: meningioma, renal angiomyolipoma, spinal ependymoma and high-grade soft tissue sarcoma. There was no family history and hereditary syndrome. The genetic analysis was completely normal. To best of our knowledge, t...

The authors report on a 55-year-old man with complaints of intermittent pulsatile headache for 6 months, vision loss of the right eye and diplopia for 2 months. A magnetic resonance image (MRI) showed a large heterogeneously enhanced intrasellar mass lesion, occupying the entire hypophyseal fossa and extending into the sphenoid sinus. The patient underwent microscopic transsphenoidal surgery fo...
متن کاملMetastatic lesions involving the sella: report of three cases and review of the literature.
Metastatic cancer must be considered as a possibility for intrasellar masses. Newer treatment modalities, such as gamma knife radiosurgery needs to be explored for these lesions. Three cases of intrasellar metastatic lesions were retrospectively reviewed. Presenting complaints, radiographic studies, operative procedure, and histopathological confirmation were recorded for each patient. All had ...
متن کاملAggressive Supratentorial Ependymoma, RELA Fusion-Positive with Extracranial Metastasis: A Case Report
Ependymoma is the third most common pediatric primary brain tumor. Ependymomas are categorized according to their locations and genetic abnormalities, and these two parameters are important prognostic factors for patient outcome. For supratentorial (ST) ependymomas, RELA fusion-positive ependymomas show a more aggressive behavior than YAP1 fusion-positive ependymomas. Extracranial metastases of...

Stereotactic gamma knife surgery (GKS)-induced brain tumors are extremely rare, and no ependymal tumors induced by GKS have been reported. Therefore, little is known about their clinical, pathologic, and genetic features. In addition, a regimen of adjuvant chemotherapy for anaplastic ependymoma (AE) has not been established. A 77-year-old man presented with a gait disturbance and left-side cere...
